By completing this registration form, you acknowledge and agree that your personal information including your name, address, phone number and email address (Personal Information) is being provided to The Pharmacy Guild of Australia, Queensland ABN 87 076 197 623 (QLD Branch). The Personal Information will be used by the QLD Branch to administer and manage your registration to Pharmacy Connect, and to keep you informed about associated industry events, offers and services available in community pharmacy. 
 
The QLD Branch is a Branch of The Pharmacy Guild of Australia ABN 84 519 669 143 (the Guild) and, as such, we may disclose your Personal Information to the Guild, to Guild Branches, to the Guild’s and Branches' related bodies corporate, and to agents, contractors, service providers and partners engaged by the QLD Branch or the Guild in connection with Pharmacy Connect. The Pharmacy Guild hosts the Australasian Pharmacy Professional Conference and Trade Exhibition (APP), held on the Gold Coast each March .  If you wish to receive information on APP, please indicate below:

A separate registration form must be completed for each individual attending the event.
